I used to think that when parents were discussing what to name their child, they had to do over the table at mealtimes, so the first, middle and surnane would be like breakfast, lunch and dinner, or starter, main course and dessert, with the baby name book as an imaginary menu to choose from (you get the idea, when certain subjects make you hingry for no actual reason).
My mom made a song when I was little called Chrissy "Bear". I got very confused and used to think and tell people that my last name was Bear. I insisted that it was and my pre school teacher had to call my mom to ask if my last name was really different then hers.
I used to think that my middle name was actually, because my mother was always forgetting the names of her children. She would say "Suzi actually Alice could you please come here"

I used to believe that people with certain combinations of names, like "Carla" and "Melissa", for example, or like "Tammy" and "Lucy" were mutually invisible to each other, couldn't interact with each other in any way, and would be totally unaware of each other's existence. But then later I knew of a Carla and a Melissa who were each other's best friends. So they clearly knew about each other and interacted with each other. So after that I gradually got over that belief.
When I was like 3 or 4, I thought people were born with their name. For example, if you're name was Jenny, when you were born, you had a little sign around your neck with "Jenny" written on it! Don't ask me where I came up with this. I was a weird child.
When I was a teenager, I used to believe that someone's name-day occured as half-birthday.
i was totally shocked when i heard some kids saying "nanny,nanny poo,poo you can not catch me". i was like "how do they know my name and why do they want me to catch them,i have never ever seen them before?"i then started yelling at them thinking that they were teasing me.now some people call me poopi instead of nainie.
I used to think that all people who were called "John" were actually named "Jonathan" and "John" was just the shortened form of the name. Same thing with "Chris" and "Christopher", as well as "Alex" and "Alexander".
When I was younger, I had a chinese name and a christian name (which was really more of a nickname). When my brother was born, he didn't have a christian name and I asked my mum why. She told me it was because my grandparents had died, and because of that, all the kids in the family who were born after the grands death couldn't have christian names too. I believed her until I was 20. can't believe I was that stupid.
I used to think my mum and dad were brother and sister. How else would they have the same surname?
When my sister and I were little, we used to play "friend". One of us would walk up to the other and say something like "hi friend" and we would go back and forth talking completely normal, except we used the word friend instead of a name. We thought that's how you talked to your friends!
When my mother used to talk about Princess Diana, she usually referred to her as Princess Di. I thought she was talking about some sort of evil vampire (Princess "Die") and wondered why people always mentioned how kind and wonderful Princess "Die" used to be.
I found out years later that Di was short for Diana, which made a lot more sense to me! ;)
I used to believe that everyone who had the same name, looked alike. E.g. my father's name is Harold, so everyone named Harold looked exactly like my father...
When I was a kid in elementary school, I saw the title "Madam" on a door in the school hallway. I didn't know what it meant, so I assumed it was a man's name, like "Adam".
I thought that the teacher in that class was a man like my dad. I got disappointed when i took french class and i saw a french woman as my teacher instead of a cool man. I got so angry and disappointed that i almost made her cry!
I used to think that every country had a queen or king, and I thought that George bush was the king of America.
When I was little and my sister was just learning to write, I taught her that the proper spelling of her name was Katiewxyz. She spelt it like that for quite a while.

My brother's name is Benjamin, so he was always called Ben. Whenever my mom was mad at him, she would say, "Benjamin (last name)!" So for wuite a long time he believed that his middle name was "German".
